Health and safety form: Maddis Bedroom

Location and planned day of shoot	Maddis house on the 10th December.
What equipment will be taken? Who will be present? What props are required?	I will need the camera, tripod and script. Will be myself and Maddi present with no props required apart from costume.
What are the potential hazards associated with the location?	No potential hazards.
Is there water at the location (pond, swimming pool, waterfall)? How will this be planned for?	No water at location.
What are the risks associated with the general public? How will these be dealt with?	No contact with general public when filming.
How will the equipment be stored during the shoot?	It will be stored safely in Maddis bedroom.
How will the equipment be stored after the shoot/overnight?	Stored securely in my house.
